For guidance on making end of life provision, we particularly like the book Last Orders, by Patricia C Byron, which has sections to fill in that guide you through leaving a letter of wishes detailing funeral arrangements and other concerns.

Last Orders ..(will) offer your family and executors clarity at a time when they will be needing assistance. Giving your time to the completion of this document now, will help your loved ones in the future. This book is, quite simply, exceptionally useful.
About the Author
Her book Last Orders was borne out of necessity. When she was asked to be Executor of her friend s Will, she had no choice but to think quickly. Her friend was terminally ill and lived alone so Patricia needed vital answers to difficult questions. In late 2006, just a few months before her friend sadly passed away, Patricia wrote Last Orders to assist them both.
However, that is not the end of her story. When her mother died suddenly, Patricia learnt even more about the process of handling estates and dealing with all the challenging aspects surrounding death. Her mission is to incentivise us all into leaving clear, unambiguous instructions about our final wishes.
She now presents talks and seminars on the importance of getting one’s affairs in orders.
